Data Catalogue

Indicates whether the individual has access to the required technology to support care.

Access to technology

Information describing an individual’s living arrangements or housing situation. This may include the type of residence (e.g. private home, supported housing, care home), tenure status, and other relevant details that provide context about where and how a person lives. Accommodation data is often used to support service planning, eligibility assessment, and the delivery of health and social care.

Accommodation

Free text field for recording any additional relevant information not captured elsewhere.

Additional notes

The location details for the individual, including residence or place of care.

Address

Information about known allergies or adverse reactions to substances.

Allergies and adverse reactions

A structured plan outlining the individual’s care needs and agreed interventions.

Care plan

Records whether the individual has given consent for their information to be shared.

Consent for information sharing

Information required to contact the individual (e.g. phone, email).

Contact details

Details of the individual’s current health or care conditions.

Current conditions

The calendar date on which an individual was born, typically recorded in a standardised format. It is a core personal identifier used to verify identity, determine age, and support eligibility, legal, clinical, and administrative processes. The date of birth is often used in combination with other personal attributes to uniquely distinguish an individual.

Date of birth

The date on which the individual was referred into the service.

Date of referral

Details of any device issued to the individual for monitoring or support.

Device provided

Contact information for a person to be notified in case of emergency.

Emergency contact details

The individual’s ethnicity as recorded for care purposes.

Gender

Summary of the individual’s overall health and wellbeing.

Health and wellbeing status

Height

Information about the individual’s current living situation.

Living arrangements

Details of medications the individual is currently taking.

Information about the individual’s ability to move independently.

Mobility status

Monitoring data

A unique identifier assigned to an individual within a national healthcare system to support accurate identification across health and care services. It is used to link records, enable safe information sharing, and ensure continuity of care across organisations and systems. The format, governance, and issuing authority of the identifier vary by country (e.g. CHI Number in Scotland), and it is typically managed in accordance with national data standards and privacy regulations.

National Healthcare Identifier

Details of the individual’s next of kin.

Next of kin

Clinical or care observations recorded for the individual.

Observations

The family name of an individual, typically shared with other members of their family and used to identify lineage or familial relationships. Also commonly referred to as a “surname” or “last name,” it is recorded as part of a person’s full legal name and is used in administrative, legal, and identification contexts. The format and ordering of the family name may vary across cultures and naming conventions.

Person Family Name

The name assigned to an individual at birth or through legal naming processes, used to personally identify them within their family or social group. Also commonly referred to as a “first name” or “forename,” it distinguishes an individual from others who share the same family name. An individual may have one or more given names, depending on cultural or personal naming conventions.

Person Given Name

The language the individual prefers for communication.

Preferred language

The main condition for which the individual is receiving care.

Primary condition

The origin of the referral (e.g. GP, hospital).

Referral source

Assessment of risks related to the individual’s condition or environment.

Risk assessment

The date the individual began receiving the service.

Service start date

Information about the individual’s social situation.

Social circumstances

Details of the support the individual needs.

Support required

Specific needs that can be supported through technology-enabled care solutions.

Technology enabled care needs

Key physiological measurements (e.g. blood pressure, pulse, temperature).
